I think it's (over-scrupulous) fear - Fear that someone will notice them never using a urinal (even though plenty of guys who aren't in a hurry will sit down anyway?) and that this will be the one thing that stops them from passing.

I'm going to take a wild guess based off reading this thread: the docs who do that kind of thing likely approach it as a rare, even once-in-a-lifetime chance to flex their skills and put themselves on the map, with a team of urologists, actual plastic surgeons, etc, knowing that respected people in their field are paying attention, working on an otherwise mentally/physically healthy man who suffered terribly and also might have something left to work with as you mentioned. And I'm sure it costs a FORTUNE.
This is in contrast with: opportunistic docs who aren't actually skilled in plastic surgery or any other specifically helpful field, who are accountable to next-to-nobody, who set up exploitive private practices raking in medicaid bucks from mentally ill self-destructive Aydens who have no recourse because instead of wanting their mutilation repaired they're begging to be mutilated. No proper wound care, no psych consult, no answered phone calls.

I believe it, because I think a lot of these doctors are sociopathic maniacs. But can you imagine having one of these troons as a patient?? It looks like that the healing process went horribly wrong, and for some reason the left side migrated/scarred upwards, rotating the entire fake dong and causing it to flatten and malform. The seam that’s visible in the “under the flap” pic is clearly meant to be on the bottom of the sausage roll.

My completely unqualified guess is they got REALLY bad wound separation on their left side of the sausage roll, and when it finally healed it pulled the sausage up.
