Likely due to being pre-diabetic as a result of insulin resistance. Type 2 diabetics often develop skinny arms and legs and just have trunkal obesity. Diabetic sarcopenia occurs due to damaged microvascular vessels as a result of high blood glucose. Muscle weakness and wasting occurs when muscles have inadequate blood supply. Diabetic neuropathy also develops which impairs mobility in conjunction with obesity.it's really odd considering how skinny his arms and legs are.
His chronic alcoholism is also wreaking havoc on his pancreas along with his shit diet. Wouldn't be surprised if he ends up having to take metformin soon if he isn't already on it.
