Likely due to being pre-diabetic as a result of insulin resistance. Type 2 diabetics often develop skinny arms and legs and just have trunkal obesity. Diabetic sarcopenia occurs due to damaged microvascular vessels as a result of high blood glucose. Muscle weakness and wasting occurs when muscles have inadequate blood supply. Diabetic neuropathy also develops which impairs mobility in conjunction with obesity.

His chronic alcoholism is also wreaking havoc on his pancreas along with his shit diet. Wouldn't be surprised if he ends up having to take metformin soon if he isn't already on it.

Id say he's past pre-diabetic and is low level untreated diabetic with how high his sugar intake is.

If Ronnie had some solid genetics that he passed onto Ethan before he croaked, then Ethan will miraculously live past 40 before his organs slowly start to give out from the constant substance abuse. He's definitely not going to make it past 50, though.

He's clearly got some sort of alcohol related psychosis and all of the problems that are associated with it. If he's going through a fifth of liquor daily for years he's probably going to need serious rehab to get off of the stuff since withdrawal will likely kill him.

If he gets a physical he might be confronted with the truth about how much damage he's done over the years. His body can't take his addictions anymore and it shows.

Alcohol related psychosis is pretty crazy. It’s not as if people can just stop they need to be weened off of it. I can’t see Ralph ever admitting it and seeking help - he knows he has health issues (Skype leak) but has not shown any interest in solving the issue.

They real big problem is the numerous physical and psychological issues that have ahold of him are all twisted around each other at this point. Apnea, diabetes, high blood pressure, alcohol abuse, drug abuse, etc all play on each other and strengthen one another. Fixing one can easily just hit the gas pedal on another.

Ralph hasn't posted or gloated about losing weight recently meaning he's either stagnated or has put more back on. so I'm gonna assume it was a one or two week thing.

There's probably nothing I want more in life (that's directly out of my control) than a blood work and body fat percentage in regards to Ethan Oliver Ralph.

His body composition would bewilder modern physics even. How can so little muscle mass support so much weight? Ethan could be the key to limitless energy creation!

The morbid curiosity is too much for me to handle at this point.

Judging by his height admitted weight and size of his arms and legs i would guess 37% body fat and a bac of .17

You need a lot worse than just fatty liver to be puking up black shit. Fatty liver disease is actually decently common, and ranges from very mild to severe

According to this, non alcoholic fatty liver disease now affects one in 3 adults:


Of course he would have the alcoholic version, but you still need more than just a fatty liver

I really don't think Ralph is going to die within the next few years- the human body can put up with an impressive amount of abuse. Lemmy, lead singer of Motorhead, apparently drank a bottle of Jack Daniels every single day from the age 30 until the age of 70- as well as keeping up a mind-bogglingly expensive meth addiction.

Living longer than expected, however, won't be pleasant for Ralph though. If anything- it will be an existence of prolonged pain.
