I’m looking at this again and I see a few things.
1. Why is there an incision on the thigh? There’s no reason to have one, yet there’s a clear incision at least 4cm or so long, which looks like it’s been kept wet
2. Look how messy the incisions are. The left side of the scrotum has that flap at the top, like it was hacked apart.
3. I can’t see any stitches or mess where stitches have burst and fragmented an edge. Maybe two at bottom right, but I’d expect to see stitches no? There’s certainly no layers of stitches.
How long post surgery is this person? Why is there a huge cut on the thigh? Why does everything look like it’s been kept underwater? So many questions

This might (to be clear, there is a ton of guesswork in what I'm about to present) be a PR/rebranding move.

Based on a quick search of Pennsylvania's medical licensing page she is still licensed in that state with no disciplinary action or ongoing state level civil litigation. The expired licenses appear to be from when she was still a medical student and when she had a physician's assistant leave her practice (not a doctor or lawyer but the assistant's license was terminated around the same time as Rumer's, and since they both got relicensed there is no reason to believe it was for disciplinary reasons).



Now for the interesting part: Google Maps still lists her as open (suspect since it still gives the obsolete URL for her practice but with the sheer volume of disgruntled patients Rumer has, they'd probably update it to 'closed' if they had any proof imo which might say more about Google being slow to update since the inactive website is still linked):


But two of her licenses were taken out early last year with an address in Conshohocken, Pennsylvania (I only included the one for her surgical license, there is a functionally identical one that lets her dispense controlled substances):


So maybe, just maybe, she is laying low and moving her practice to try and ditch the 'Butcher of Ardmore' label that triggered a libel lolsuit a few years ago. We know she hired a PR firm to prop up her reputation after all the Internet backlash..... and it sure hasn't helped since a private DuckDuckGo search for her name turns up less-than-flattering associations:
