Could be multiple things, if they are actually in pain, and not just pretending.
Hormonal imbalance from the HRTs, unhealthy diet causing ulcers/IBS, or if they got the surgery, their gaping wound (which is also prone to infections).

Not even a joke, sometimes I come across a female British Youtuber (it's usually horror or politics) and I get the urge to ask in the comments whether they're trans, or just British.

She should switch to Aussie Broadband, who actually has English-speaking customer support and is very worth the extra cost.
Not to mention that most Aussie ISPs blocked 4chan, 8chan, and iirc the Farms, for the 6 months following Christchurch. Aussie Broadband didn't.
