If you even lose the terminal segment of your index finger, a lot of piano repertoire will be off-limit to you. But such people got on with life, and they come to term with their disabilities and dashed hopes and find their self-worth in some other pursuits. It won't be easy; they need a lot of help. For the case of "Gelly", that would entail learning to accept that he will never be a sex-having woman, but that doesn't mean that life is devoid of meaning. A lot of doors are still open, and a therapist should be there to help him realize them. If he still likes men, he can try to cultivate a companionable, non-sexual relationship with them. Unfortunately -- and this is the ultimate cruelty and injustice -- no therapists will offer the help he needs, for fear of being tarred and feathered as "transphobic" and practicing "conversion therapy".
