At least where I live in the UK, a lot of public disabled toilets are kept locked. I think to stop junkies shooting up in them or homeless people moving in or other degeneracy. You can buy a radar key online that lets you into these toilets. I don’t think you have to prove that you are disabled or anything but I’ve never looked to buy one.
