This is exactly it. Ralph truly is an addict, not just with substances (though obviously he's addicted to those too) but with gratification/dopamine. He's a manchild who has never really had to take care of himself, let alone anyone else and all he knows is gorging himself on whatever he wants.

He had a wife but they didn't even live together. He lives/lived with his mother but can't be bothered to get her trips to her dialysis appointments when he's drunk/stoned. He has a pregnant girlfriend and will soon have a child, neither of which he gives a shit about taking care of. And he was a raging alcoholic/drug addict through it all. Ralph is a massive failure of a man in all respects. Even when he went to jail at least his room and board was paid for. He's never had a legitimate job. He's a loser who has never had to be an adult in his life. This latest episode with the threesome idea is just further proof, as if we needed any.
