Last I heard, it's medically required. But then again, I don't remember where I heard that.

Edit: It is an requirement. Otherwise the body will think it's an wound and try to close it. Plus the process takes around two hours a day....So, yeah. It ain't natural, obviously.

Ugh I just remembered that there was product placent for Dilators in I am Jazz. After srs Jazz had to hold the dilation rod up in the air and Amir visibly cringed.
Later the dilation rod company made a twitter or instagram post (can't remember) about it.

I really wish Gender Dysphoria is still in the DSM, because there’s no way that wanting to chop your perfectly good peen/tits off is considered normal in any way. Also, as a result of the DSM trying to be WOKE with the times, I feel as if this “normalization” of dysphoria’s only made it worse for people with actual dysphoria; among the ranks there are transtrenders, and autogyneophiles that make up the more “vocal” activists so any sort of progressive discussion about the harmful side effects of SRS will be sugar coated.
On top of that, does SRS even help with dysphoria? It seems to me that troons regardless of their genital mutilation status still experience the occasional bout of dissociation, so what’s the point of a pricey and potentially harmful surgery?
I mean, in my uninformed cishet opinion I think there should be more of a focus on a mix of therapy and meds to help qualm dysphoria and hopefully make the person feel more at home with their bodies because surgery and hormones have long lasting effects you can’t back out of once you start, and it doesn’t seem like neither would fully cure dissociative attacks. But any sort of research on that front would be considered transphobic

There isn't any broken skin, which is what is generally required to be considered an open wound, but there is an area of the pelvic cavity hollowed out and then skin is stuffed up inside it to line the interior. Having to tack up the apex of the neovagina is one of the more common revision surgeries since this is a really unnatural rearranging of flesh and the uppermost part of the neovagina is a natural point for failure to occur.
Keeping an open wound dilated by mechanical means is more like having your ears pierced and keeping the earrings in until the scar heals. If it worked like that then it would be healed well enough to not need dilation anymore, but they need it permanently (although on a less intensive schedule than was needed initially). From what I understand the neovagina will only heal shut if the pressure from the pelvic cavity forcing the skin together isn't interrupted by dilation.
yes. A troon in prison sued the state because they were denied dilators while in detention and their neovagina sealed up. They won a bunch of money and IMO they deserved it.

The inverted penile skin isn't the issue- like the ear piercing metaphor upthread, once the skin heals it can stay like that more or less indefinitely. The issue is that men lack the "sling" of muscles and ligaments that women have cradling the opening to the vagina. In men, those muscles run perpendicular to the penis.
Surgeons have to slice through the musculature to create an opening, and body is forever trying to heal those muscles and ligaments back together. If the opening heals shut, the mtf is left with an inaccessible skin cavern filled with sweat, pubic hair and smegma, and no way to clean it out with anything larger than a qtip.
