If I had to speculate, I'm guessing it's the whole slapfight with TCS and that Kairi troon that Kevin got involved in. Seems like it was a major shitshow.
I think he's talking more broadly about Bluesky.
It launched two years ago but for the first year it was invite only, so you had to know someone on the inside to join and so it was a super special secret club (especially for trans people "fleeing" twitter). They opened registrations last year, and also made all Bluesky accounts public. You can limit interactions to just people you follow (although you can't limit to "only followers") but you can't hide your posts from logged in users, so Kevin has lost the ability to get involved in stupid drama and then private his account to hide for a few hours. Best you can do is hide your account to people who don't have a Bluesky account (which Kevin does) but that's meaningless now anyone can register.
They've (basically) lost their special Shinigami Eyes plugin that highlights trans friendly stuff and warns about transphobic stuff which was a fun powerplay for them. Now instead there are "community moderation lists" that you can sign up to, which will filter out wrongthink - but it's broader in scope and there's multiple versions, so Kevin needs to be a bit more tactical with what he posts because if he ends up on one of those trans community moderation lists (or muted on a curated "Skyfeed"), his reach will be severely limited.
And I think that's the main issue for him. He was trans famous on twitter and that stupid tweet of his kept feeding people to him through the algorithm - but on Bluesky he's only got 3.3k followers. He doesn't get the engagement on there that he craves, and most of the people he'll organically interact with are just like him, so he can't own the transphobes or find anyone to buy him Ko-Fi (since everyone who reeds his "skeets" is broke). Few of the weird porn artists migrated over to Bluesky so he doesn't get that in his feed as much, and lots of his followers either didn't follow him over, or didn't bother to follow him once they migrated over. Plus while he might tweet out "

HORNY FOR GODXSS", there's obviously a lot of weird and cringe letching going on in his DMs - but Bluesky doesn't
have DMs, so if he wants to communicate with someone he either has to do so publicly or at least publicly contact them asking for a way to contact them privately.
Basically it's a platform heavily moderated to exclude transphobes that emphasises transparency and accountability and builds a smaller network. Now the novelty has worn off, he can't admit that's actually the worst idea for a platform for him without admitting there's certain things he doesn't want sunlight shone on. Hence the coping he's just skeeted out:
