How long do you think it will be before he realizes the majority of those teeth are probably going to be extracted? I've heard stories about druggies/alcoholics who show up at the dentist after like 2-3 years and lose like 7 teeth. Considering how dulled his tastebuds are, he could probably barely feel the rot until recently. Is Clint going to pay the 2,000+ to get all his teeth pulled?

Spent the past week or so investigating this, I’ve come away from it both exacerbated and ambivalent.
A few hours after the dentist has already pulled them.
Clint just drives him to walmart to get some more duster as a treat for being a big boy at the dentist.
Oh, for sure this wont be a one time thing. As you said he needs at least a couple rounds of heavy antibiotics for the periodontal inflammation. They will then probably pull what they can in two seperate visits. He would then need another visit to debride and clean the jawbone under his gums. Another trip to add bone grafts to help give his jawbone some structure, and then he might possibly be able to get some dental implants if the bone grafts adhere properly. This is going to takes months and tens of thousands of dollars.
I think you're right but he ain't doing any of that shit. At most he will get his top 4 front teeth pulled and that is it. But even more likely; they will just be left to rot and break down in his mouth. He is incapable of any kind of aftercare to properly care for extracted teeth, bone grafts, implanted posts, crowns or bridges.
Clunt will have to tell him that teeth don't grow back. After the extractions of course. Those root caries he has on his incisors are very bad. His most prominent teeth are doomed. It's kind of sad. I almost feel bad for bog. Almost...
I almost can't see this playing out any other way than Josh just gradually spitting out his teeth on stream like Onlyusemeblade and not giving a single fuck or playing it off like he's a badass for losing his teeth. Especially that he's doing duster now, which I imagine will quickly destroy what scraps are left of his brain and turn him into an absolute ghoul.
