Kinda doubting that those are self-inflicted instead of just from her many cats, but even if they were... Ralph hasn't been in a relationship with a mentally healthy woman since Nora. Highly doubt they're self inflicted scars. As others have mentioned they're in a hard to reach place, but one thing to consider is that they look relatively deep, it would be a pain in the ass just to barely scratch yourself there, let alone use enough force to leave scars like that. Also, other than it being a hard place to reach, it just doesn't make sense to have in such a random spot. You can't really say she hates her shoulder or some shit, and if it was for pity points it would've been on the wrists or another highly visible spot.
