A shadow that kind of looks like a fatroll (when zoomed, but it's clearly just a shadow) and a couple of blemishes that might be scars.
People have scars, and if they were self-harm scars, there'd be a lot more of them and they'd be way shallower. Anyone who has ever known a cutter knows that. Ralph (who has never had a real-life friend, much less a female one) does not.

Is Ralph insinuating that Venti cuts herself? As someone who, when she was younger, did self harm I don't think that Venti is a cutter or at least cuts herself on her upper arm. Most girls when they cut themselves usually cut on their lower arms or thighs. Places that are easy to reach with a knife as well as easy to hide the cuts and scars. Even now I try to see how Venti cutting herself on her upper arm would work and it is quite hard to try to cut yourself on your upper arm. My guess is that those scars are from Venti's many cats.
Also, Ralph should be the last person to make fun of someone's appearance given that his body looks like that of the archdevil Baalzebul from D&D in his cursed slug form and he is the only person I know that has 4 tits.

How would she cut herself at the back of her upper arm? Those cuts are deep enough to leave scars. She'll have to pull out the knife wrap her primary arm all around just to reach that portion, she'll have to awkwardly twist her neck to see where she wants to cut and literally push inwards to inflict such a wound, unless she's using an extremely sharp sashimi knife or a fucking microtome knife.
All of this is made even harder because of her massive breasts, something Ralph has as well and can really sympathise with. I doubt Ralph can even reach the backside of his arm from his other hand.
Those do look like deep cuts, maybe one of her cats got mad or it was an accident when she was younger. I know a couple cutters, they do it to their thighs and lower arms.
