Lung transplant exists, but chronic smokers with severe COPD are usually fucked in multiple organs systems and might have occult cancers as well, so they are not usually considered. The usual candidates for lung transplants are patients with pulmonary hypertension and pulmonary fibrosis.

The thing with most transplants is that people need to be mostly healthy (except for issues directly related to the organ that is failing) in order to even get considered for a surgery. If you get even something really stupid like a cold while you are on the transplant list your name will be suspended from it until you are healthy again.
Troons arent healthy and dont take care of themselves, they are terrible transplant candidates. Its even worse for gruffin cause she has lupus, and afaik the disease needs to get to a stage of remission before any type of transplant, even if you ignore the drug addiction and other stuff she has going for her.

That's one of the many weird things that goes on with the transplant list: you're there because you're dying, but you can't be so sick that you can't survive the surgery. It's like the doctors are Miracle Max from The Princess Bride: there's a difference between mostly dead and all dead. Either way, they're taking all of your spare change.

Lung transplants are absolutely a thing. They’ve been done since the sixties.
For some people, they can take a lobe from the lungs of two living donors and replace both lungs in the patient with the lobes. They can also do a single lung transplant from a brain-dead donor (meaning one donor can give a lung to two patients.) For patients with cystic fibrosis they must do a double-lung transplant.
Long-time smokers aren’t likely to get new lungs. Overall health is a factor, and too many smokers suffer from co-morbid conditions. No organ transplants are done for people in active addiction. Complete abstinence from substances is required for a minimum duration before approval to go on the transplant list. Even then, if psychological assessment indicates the person is likely to resume use after the transplant, they will be refused.

Most people aren't aware of the trade-off of organ transplants. It takes away the chronic illness you're dealing with, and gives you a different chronicity: immunosuppressed transplant patient.
It's not necessarily a bad tradeoff, but beyond scarcity, that's the reason organ transplants are reserved for end-stage illnesses, where being a transplant recipient is a trade up.
