Sensation from Dr. Freet's Abdo + RFF Nerve Graft (see comment)
I had abdominal phalloplasty with Dr. Freet in March 2021. Initial surgery was just penis creation. In August of 2021 I had urethral lengthening and nerve hookup through the RFF nerve taken from my arm and inserted in my penis during UL. Dr. Freet stated that this should result in higher erotic sensation than regular abdominal although he didn't expect to change tactile sensation too much. If that sounds confusing, it kinda is. Erotic sensation is already a highly individualized sensation to try and measure and to compound it erotic feelings can be entirely mentally dependent. Having said all that I wanted to share my general experience with sensation post nerve graft because it has been just shy of 2 years since my stage 1 and 1.5 years since my stage 2.
In the pictures I shaded entirely based on tactile sensation, green is feeling, red is not. I won't try to rank the areas i feel better or worse, but I will say the sensation on my right side is far more robust and definitely capable of feeling erogenous in some spots nearer the base. There is also deep tissue sensation in some of the red zone when I squeeze but it is very inconsistent and hard to quantify in a measurable way. I have not had the easiest time connecting to my penis from an erotic standpoint, glans was always a must have in order for me to and I still haven't gotten it done. But having said all that I still masturbate using my penis to stimulate my buried tdick so I don't consider it a hindrance. If you couldn't tell the nerve hookup is on my right side and my sensation is hands down better over there. Prior to stage 2 with the nerve hookup I only had sensation in a ring of skin around my base and my penis was entirely numb. Sensation growth has been gradual forsure and I have no idea where it'll end but I know people can continue to gain it even years out. I gained most of my sensation immediately after and within a couple months of nerve hookup, but 6 months to the 1.5 year mark I've gained .5 of an inch of sensation on the right side and .25 of an inch on the left. My total length is still at 5 inches, with the exception of immediately post surgery when it tends to come up shorter I've basically stayed at this length since stage 1.
My overall verdict is that while I do think the nerve graft helped a lot, I don't think it panned out the way I expected. Part of the issue is perhaps my own misconception. I went into abdominal phallo under the impression that tactile sensation halfway up the penis was guaranteed because the nerves are never severed. I later learned that not only is this not guaranteed but that its also fairly common to wake up completely numb after abdo rather than having sensation right out the gate. This was my experience, I lost all sensation over the entire area, both my penis and stomach after stage 1. I do believe that if not for the nerve hookup boosting the right side I'd be at ~1-1.5 inches of sensation on both sides and the feeling on my right side would not be as strong as it is, but its always felt as though I was behind the curve on progress which is something I've been working at accepting and no longer judging my body on. So in comparison to my pre op expectations this is barely on par with what I thought I'd have regardless, but in comparison to where I started at, this is leagues better and im happy for that.
Final disclaimer, please don't take my post to mean that this is the only sensation experience anyone will ever have with this method. I'm a single case and it is quite possible that someone else had a better experience, the same that as it is that someone could have had worse. I mostly wanted to put this out there because it's hard to hear about sensation perspectives a year+ out from surgery, especially on relatively new phalloplasty methods such as abdominal + nerve graft. It's ah, not exactly pain but dear god is is an incredibly uncomfortable experience.
