Infection sure, but in addition to the agony that is moving while impaled the point of the dilation is to prevent healing while, somewhat counterintuitively, encouraging it.

Nevermind arousal (impossible) but there isn't even a sense like "whoa! That person has no clothes on!" that you'd feel if you saw your neighbor streaking down the road, or opened the wrong hotel door, or something. Even with the completely healed pics.
Even a medical photo with a nude person in it would look more "naked," which is weird, since that is essentially what these are. Idk, maybe it's because their genitals have been so obscured, so obliterated, that it's often more like looking at (clothed) victims of a landmine than looking at someone who is actually nude.

I suspect the wound would try to heal around the plug and possibly even absorb it in. It's not a vagina; it's an open injury the body is trying to heal.
The vaginal plug is called a "pessary". They're made in different shapes for different issues. In addition to the healing issue and risk of fistula, I wonder if there is just too much variation in neovaginal shape for a pessary to fit. They'd have to be custom made, and we know that troons are lazy.

The body really doesn't like foreign objects in wounds and besides the infections, the scar tissue would be a beast. It's possible that the skin would try to heal around it, like trees that grow around buildings and whatnot. Try to imagine what it's like to stick a toothbrush into a bullet wound and keep it there indefinitely. You'd get very similar results.
