WHY IS IT SO BLACK

It's a vacuum-assisted closure system, aka wound vac. The black is a foam that fills a wound or goes over an incision; it's porous but sturdy enough to allow even distribution of low-level suction, instead of having the suction just fwwwp the closest wound-meat into the end of its tube.

Black is the most common color of foam under a VAC dressing, but you'll see grey (silver added), white on skin grafts, and there's a special mini-VAC with purple foam dressings that reinforces surgical incisions and gets sent home with patients a lot, because it goes over the normal sutured closure.

He caught my attention because he posted a question on r/transgender_surgeries about whether you can revise a sigmoid colon neovagina with amniotic membranes.

Another TiM asks if his neovag is failing and he responds saying it hasn't but he needs to undergo a colectomy because of the procedure.

I haven’t lost it, I need to undergo colectomy to resolve chronic constipation and rectal prolapse that resulted from colon SRS.

I guess we can add chronic constipation and rectal prolapse to the list of issues SRS can cause.

He hasn't posted much but he went to Thailand for surgery about three years ago and went to Kamol. He also did 150 hours of electroysis and says it barely made a difference.

I'm thinking now. Jazz Jennings's rot pocket at 17 spectacularly fell apart within hours of surgery, that patient that died shortly after surgery in the Dutch Protocol studies was 18. I assume this person is a minor or considered one for insurance if he is going to the Children's Hospital. Guess he was on puberty blockers like Jazz and the Dutch kid were?

This gave an odd thought. Rotdogs don't give hair balls, but can you imagine kidney stones? Certainly the bladder, urethra, and kidney tubes aren't "designed" for it, but the tissue is fairly strong and capable and used to a lot of dangerous material moving through. A floppy skin sausage probably isn't.

I guess it depends on what they use specifically for the actual neourethra. Do they loop the skin back around internally for the neourethra, like those gel tube toys from the 90's? (@Otterly beat me to it)


If so, then it can probably handle some abuse since it's just external skin too.

Any junctures are probably weak though. Or maybe the place where the neourethra meets the old urethra might leave a corner where stuff can get stuck.

The problem too is the length of it. Women’s urethras are really short and our bladders are as strong as they need to be to get urine out. Men’s need to be stronger to get it out of several inches more of piping. When you have stale urine sat against skin it leaves burns, and these urethras are all at an odd angle so urine WILL pool, it’s just how it is - it’s like the u bend on a loo. So you add structures and fistulas and a bladder too weak to push all the Horne out and it’s going to be a burned, scarred and inadequate mess. Kidney failure is what’s going to kill these girls down the line.
Several cm more I mean, not inches.

Update 2 (Info from Dr Frances Grimsted)

It was a complication. Seems like the stitches were rushed and a ton came undone. I dont have any walls to the first bit of my vaginal canal but she assured me that it is a fully functional vagina.

Next lets talk about the skin flaps at the top. Those were what covered the holes and were secured by the stitches. When the stitches let go those skin flaps went up to their resting position. The left flap (if you're facing the picture) took all the tissue under it with it. The right flap had healed a bit more and so the tissue beneath stayed behind. The yellow bits are discharge (totally normal) and the black bit near the vagina is indeed necrotic tissue. It will be removed in a few months once its safe to do so. The line of tissue left behind by the right flap is most likely going to tear. But new growth will replace it.

At the end of the day: keep dilating and stay away from Rumer.

Why the cut on the thigh? It looks like there were no stitches at all in it. A rurally I can see a couple at the top right, but wtf was the closure for this? It also looks as if insufficient tissue was left to cover the area ao it’s been pulled too tight and as soon as he’s moved his legs apart it’s all just ripped. Absolutely horrific. Total butchery.

Ok I'm not a doctor or anything but is it really safe to leave necrotic tissue attached for months. I don't even understand how that can even happen, I though debridement was essentially required immediately to prevent infection.

Babysoots OP: I went to an OBGYN trained in gender affirming care. Her team assessed the damages and took very good care of me. It has been cleaned and we were given instructions and resources on how to keep the wound clean and help it heal.

OBGYN trained in gender affirming care... Exactly who you want to go to when you have wound separation and necrotic tissue. But hey, at least your vagina is fully functioning. The OBGYN said so.
