Quite honestly, I think his brain is trying to protect him from the reality of his situation. Occasionally, you hear about people with absolutely horrific traumatic injuries who are calm and detached even as they bleed out everywhere. It's almost as though the brain just decides that if it ignores what's happening, it's not as bad as it seems. There are multiple famous examples of
soldiers who continue attacking the enemy despite suffering severe, disabling physical injuries.
This is the worst wound dehiscence I've ever seen. I think it looks indisputably bad enough that someone at the Boston Children's Hospital gender clinic is going to be obligated to take some sort of action instead of circling the wagons.
