Considering the quality of care kids have gotten in the past from actual humans, an algorithm can’t do much worse.

After spending more than Ā£1,000 over the past 15 months with GenderGP, an unregulated private clinic, Theo was already feeling less like a patient and more like ā€œa cash cowā€.

The 20-year-old student, whose name we have changed, is transitioning from female to male, and is among many of his friends desperate to access testosterone to lower his voice and achieve a more masculine appearance.

He opted to go with GenderGP over NHS transgender care, which has long queues, but is now one of hundreds of customers seemingly in revolt over their treatment.

They were appalled to discover that the online clinic they trusted as the experts in transgender medicine is now using artificial intelligence to influence their course of treatment.

The website, which is registered in Singapore, is transferring its ā€œcareā€ of a vulnerable group of people, many of whom have complex mental health issues, from advisers to algorithms. Not only that, but it has admitted in a statement to its subscribers, many of whom pay hundreds if not thousands of pounds to access off-label prescriptions for masculinising and feminising hormones, that it had switched to this new ā€œnext generationā€ system without telling them.

ā€œIt makes me angry,ā€ said Theo, who is from the Midlands. ā€œThe AI really reinforces how they’re just completely taking advantage of you.

ā€œI knew before that it was expensive and not super regulated, but it’s now spelt out to you: this isn’t like going to the doctor. This feels like a private company that just wants to make money from the vulnerable.ā€

GenderGP was founded in 2015 by Helen and Michael Webberley, both GPs, who started prescribing experimental hormone treatments to adults and children who identified as transgender the following year.

In 2022 Michael Webberley was struck off the medical register and Helen Webberley was suspended for putting three young patients at ā€œunwarranted risk of harmā€, but she returned to practice as a doctor after that decision was overturned by the High Court the following year.

By 2020 they claimed to have started almost 2,000 people on hormones across 42 countries. They claimed that 16 per cent of all the patients they supported were under 16.

GenderGP was already controversial as an allegedly ā€œWild Westā€ operator that continued to sell hormone treatments to people in crisis about their gender despite the lack of any good evidence of their safety or effectiveness. It also recommends that ā€œall young peopleā€ wishing to transition should have graduated beyond puberty blockers and on to irreversible cross-sex hormones ā€œby the time they are 14-16ā€, which is in contravention of NHS guidelines.

In her comprehensive review of gender identity services for under-18s, Dr Hilary Cass concluded there was ā€œno good evidenceā€ to support giving young people in crisis potentially life-altering hormone treatment.

Cass also explicitly criticised Helen Webberley in an interview with The Times, claiming her care ā€œcertainly doesn’t come anywhere near anything one would recognise as adequate in terms of a proper assessment and explorationā€.

Despite Cass’s clear warning that masculinising and feminising hormones should only be given to under-18s who meet very strict clinical criteria, GenderGP continues to evangelise about unevidenced powers of hormones to transform people’s lives for the better, insisting on its website that ā€œswitching your hormones profile from one to the other is surprisingly safeā€.

GenderGP escapes regulation in the UK because it is private. It uses doctors outside the UK, but within the European Economic Area (EEA), to sign prescriptions.

Customers such as Theo can then use these prescriptions at UK pharmacies to access controlled drugs such as testosterone without ever having met the doctor who provided them.

The switch to AI has also removed any free way of contacting the service. There is no phone number, no email address and subscribers must now pay for every single interaction.

ā€œPeople are desperate and GenderGP know that, so they can charge a ridiculous amount of money,ā€ said Theo. ā€œI know friends who have done sex work in order to get money for the fees.

ā€œI’ve been able to tutor, so I’ve been OK, but for some of my friends, especially ones with worse mental health, the only way they are able to get the money [for the hormones] is through underground sex work. It’s quite dark.ā€

To join GenderGP, customers must pay an upfront fee of £195 and £65 for an obligatory information gathering session, followed by a monthly subscription fee of £30. They can book optional sessions to discuss issues such as fertility preservation and surgery for between £45 and £150 a time. The drugs are charged separately: a three-month supply of puberty blockers costs £265. Testosterone gel costs £135.

As part of its ā€œfuture-proofedā€ service, GenderGP no longer appears to have real staff responding to queries. Instead, it tells paying subscribers to ā€œself-serve your information needsā€ and refers them to a web link for an AI-generated ā€œknowledge baseā€. The most basic interaction costs Ā£7.99 for a 15-minute ā€œlive chatā€.

This system, it promised, will be ā€œmore sofisticated [sic]ā€.

The backlash has been furious, and hundreds of subscribers have signed a petition demanding that GenderGP reverse its ā€œdangerousā€ decision to entrust their care to artificial intelligence.

ā€œThey’re our doctor for essential medicine not a f***ing video game company,ā€ said one of scores of customers complaining about the issue on Reddit.

ā€œWe (the trans community) are not a cash cow for the likes of Helen,ā€ said another.

Some said they were considering obtaining hormones on the black market, to administer themselves. ā€œI can see no other path for me than DIY,ā€ said one.

Among them is Theo. ā€œI know it’s not the smartest idea but I do think it’s getting to the point where I might have to get [black market hormones] instead because of how expensive GenderGP is.

ā€œI’ve been speaking to a couple of the guys in the gym who take anabolic steroids and have ordered some chemistry tests to see if these substances are pure enough for me to be healthy.ā€

Chemistry sets? ā€œI mean swabs my friends who are chemistry undergraduates have stolen from their labs. It’s basically just measuring for certain substances that could be harmful.ā€

Theo joined GenderGP early last year, after receiving a ā€œproperā€ diagnosis of gender dysphoria from his NHS GP. After an hour-long online session with GenderGP in which he was asked about which toys he liked as a child and whether he wore ā€œboy clothesā€, he had a 20-minute chat with online advisers once every three months.

ā€œThey looked more like student union officers, or volunteers for a helpline,ā€ he said. ā€œThe last two said they were students doing it as a part time job while at uni. They looked about my age and looked trans too.ā€

He eventually received a prescription from an overseas doctor he’d never met. He received testosterone gel through the post from Smartway, an online pharmacy based in Leicestershire, before switching to injectable testosterone, provided by another UK online pharmacy, Olympia, based near the Olympic Park in east London.

Pharmacists must ensure the prescriptions are ā€œclinically appropriateā€ for the individual, and if not, the pharmacy regulator has the power to challenge their fitness to practise or, in the most serious cases, suspend or remove them from the register.

The government is reviewing the law around what it describes as a loophole in the regulatory framework.

Roz Gittins, chief pharmacy officer of the General Pharmaceutical Council, the regulator, said: ā€œWe have made clear to pharmacy professionals and pharmacy owners that it is not enough for a prescription to be legally valid. That is just one consideration alongside others, including the responsible pharmacist’s judgement as to whether a prescription is clinically appropriate for the individual patient.

ā€œPharmacy professionals need to use their professional judgement when considering the risks to the patient of supplying or not supplying against a legally valid prescription.

ā€œIt may be the right thing in some cases to refuse to supply medication and instead signpost the patient to an alternative healthcare professional. Person-centred and compassionate communication is particularly important in these challenging scenarios.ā€

A spokesman for Smartway said that it ā€œdispenses medicines against prescriptions issued by doctors in the UK and EU, in compliance with our respective professional dutiesā€.

He added: ā€œIt is the clinical judgement of the doctor that will lead to the issue of a prescription however, we always exercise our own judgement as to whether a prescription is clinically appropriate for that individual patient. If it is not, we will not dispense unless and until we are satisfied.

ā€œWe do challenge prescriptions and have refused to accept a prescription on occasions on clinical grounds. We typically refer those under 18 back to the clinic.

ā€œA patient generally has the right to approach whichever pharmacy they like with a prescription and absent a justified reason, it will typically be dispensed by the pharmacist. All interactions by Smartway Pharmacy must be non-judgemental, inclusive, compassionate, and free from discrimination or bias.ā€

Olympia Pharmacy was contacted for comment.

GenderGP has long been unpopular among gender critical campaigners but the latest complaints mark a shift to criticism from its own supporters.

The use of vague ā€œalgorithmsā€ to assess patients’ medical histories and to recommend treatment plans, medication, blood tests and counselling was confirmed by its affiliated website, My Healthy Hormones with Gender GP.

It said: ā€œOur algorithms assess people’s medical history, their preferences and goals and aligns them with current international clinical guidelines and best practice, to help you make the best decisions.

ā€œOur algorithms make recommendations for you to have counselling or other therapy as and when required and they make recommendations for the doctor on which medication would be best for you to help you achieve your goals.ā€

In a separate message to customers from Team Gender GP, it confirmed ā€œwe’re applying bespoke software so that processes like prescriptions, blood tests and medical steps can efficiently and accurately processedā€.

Theo said it felt like a betrayal of the trans community. ā€œGenderGP appear super-woke and use all the right kind of terminology,ā€ he said. ā€œThey’re trying to be this ā€˜light’ for mentally ill youth and trying to convince them ā€˜this will solve all your problems’ but it feels like they just want your money.

ā€œIt makes me feel really scared that a lot of young, vulnerable people might get a faulty diagnosis and then do something they regret.ā€

I hate the notion of comparing vaginas(let alone natal vs neo) but let’s break this down. The typical natal vagina experiences a monthly menstrual cycle which provides a deep cleansing of the vaginal walls. Trans women don’t get that luxury or burden depending on your perspective, so naturally we have to develop our own cleansing regimens. If this is a regular routine for the lady in question coupled with a clean-ish diet their should be no foulness present. Periodt

My last job was in retail, which involves a lot of walking and standing, and while I am not sure what type of phalloplasty that my former manager got or all of the details of what went wrong, I think the complications caused her to become permanently crippled. After I left, I was told by one of my friends still working there that she was spotted in the store at the end of April, shopping. She was bitterly complaining to the cashier during check-out that she has had to have a catheter bag attached to her leg since her discharge from the hospital in December, so it sounds like the phalloplasty did not go well at all. She was apparently wearing jeans and a sweatshirt, so nobody could see what limb had been hacked to pieces, but she also seemed to move as if she was in chronic pain and one of the store employees had to carry her stuff for her to the checkout counter as well as to her car as she could not lift more than 40 pounds.

She is only in her early-20s...and yet it seems like she has completely destroyed herself, and in her attempt to be "male" she has actually made herself physically-weaker than a normal, healthy woman would be, not to mention it sounds like she has an in-dwelling catheter that she will probably have for the rest of her life.

My current position is a desk job but I do have to carry a pistol at work, and lift around 80 pounds occasionally, and I think that troons and pooners would not be able to work here, both because of the dress codes as well as the fact that nobody would take them seriously enough to hire them here in the first place as a lot of people here would have trouble keeping a straight face with the neopronouns. Plus, having a degloved arm from phalloplasty would make it difficult to steady a firearm when firing with both hands, and we cannot just let people disappear for an hour or so to go dilate.

Anyway, on another site, there's a story about a mentally disabled "transgendered" teen who is missing. This person is 17 and said to have the mentality of a 10- to 12-year-old, which begs the question: Are they trans, or is this something their parents or guardians cooked up for attention? What does he mean by 2nd operation though? What more do you have to do on a zero depth amhole? I thought the whole point of that procedure was that it was easier and there is no hole to dilate or keep open, so what more is there to do down there?

He smokes cigs and stopped several months ago however theres a shot of a Zyn container at the end. Dunno who it belongs to but nicotine is a vasoconstrictor. "As a vasoconstrictor, nicotine contributes to tissue hypoxia and adversely affects the cardiovascular system by raising blood pressure and pulse rates." This kinda thing causes skin grafts and flaps to die and impair circulation when it comes to wound healing. Its not a death sentence by any means but your risk for complications rises significantly with nicotine, even patches or smokeless. Should be an interesting ride.

It's better than smoking for healing so you have to compromise with these people. He would have been warned quitting would be the best option but told "if you can't quit, then please get nicotine another way".

Not sure if this happens outside of Australia, but at every hospital I've ever worked we have to do this dance with smoker patients who need assistance going outside (not always just post-op patients) - "No I am not going to get the wardsmen to take you outside for smoke. I can give you nicotine gum or patches. If you try to leave the ward I will page the doctor".

It's so tedious.

I had a vaginoplasty done last year in August, and at first it seemed amazing and I was super happy. But within a couple weeks, that changed pretty drastically. A few weeks after surgery, I started to have some wound dehiscence, and that separation got worse and worse and worse.

The wound separation got to the point where it was inches wide; sometimes I couldn't even find the vaginal entrance for dilation because it was all just open wound, and I was in an extreme amount of pain. I ended up being bedridden for almost half a year, barely able to walk or do anything because of debilitating pain levels. It wasn't until around 5 months after surgery that I could sit upright without significant lasting pain. It took several weeks of physical therapy to be able to walk unassisted again. My physical strength and endurance has drastically decreased as well, and I feel exhausted after even relatively minor exertions.

I was barely able to tolerate dilation for a lot of this and ended up at a dilator two sizes below the standard starting purple one. It took months to be low enough on pain levels that I was able to size up, but even 9 months out from surgery I'm still a size below what should be the starting size.

My inner labia also almost completely detached in its bottom half, and has generally just been a loose loop hanging there for the most part.

The wounded area also had a bunch of granulation tissue that needed to be burned off, so I was regularly in the doctor's office getting that dealt with. It helped, but even up until a few weeks ago there was a stubborn area of open wound that refused to close. I ended up going in for a minor surgery where the surgeon cut that inner labia loop to relieve pressure on the surrounding area, debrided some wounds outside and inside, and used a few stitches to hopefully help that wound finally close.

The vulva is also basically just a ton of loose skin at this point, and some of it is very painful. There is nothing visibly wrong with the skin other than its looseness, but there's a few spots that are sharply painful to the touch all the same. That issue is still very much ongoing, even while the other pain has mostly receded, and I have no idea what the problem even is in order to address it (if anyone has any ideas I'm all ears).

I want to note that I don't blame my surgeon for this (and for that reason I won't be naming them)- it's just one of those supremely unlucky complication situations. They and the rest of their practice have been nothing but attentive and supportive, and I get the sense they really want to help in any way they can.

It has been a horrible, traumatic year for me and it's really worn me down. I'm so tired. I just want to be able to feel comfortable in my body, but even after 9 months, I feel no ownership of my new bits. Not having a penis is great, but otherwise it's been nothing but a source of pain and discomfort. It still feels like a surgery site, not a functional part of my body. And because of all the loose skin and general breakdown, it barely looks like a vulva. I have times where I break down crying because it feels like I'm just being denied the ability to be okay in my own body. What should have been one of the most joyful milestones of my life has become the single most horrible, traumatic thing I've ever been through.

I'm not totally sure why I'm making this post. I'll likely delete it later. I guess I just needed to vent a bit about what an absolutely horrible time I've had. And I could really use some hugs and support.

RIBEIRO: Yes, it’s literally her. So you’re in a state of sobriety right now because you’re awaiting surgery, but after surgery you’re probably going to have to keep it chill for a little bit, right?


MCGUIRE: Yeah, that’s why the past two or three months I’ve been going out nonstop. I’ve just been getting so drunk and making out with so many gay men.
