Well, for a real life example free clinics in the US will often provide free diet courses and assistance, and free insulin, while doing jack shit for anything that happens to people who don't take their insulin and don't do the diet courses. That's a good example of keeping a 10,000 life saving surgery off the table with $10 medication. Although that's more to do with the blood pressure meds they give.

If you're 1000 pounds you can get completely fucked if you want a hip transplant on my dime. Druggies can get fucked. I understand getting hooked on opiates if you've had some awful back surgery, but we see people that aren't even in our system that have been weaning off heroin for 10 fucking years crashing our clinic begging for their hit and screaming that their gonna get back on heroin if we don't get back to them.
