The General Medical Council states that patients who request gender dysphoria must be referred to a Gender Identity Clinic or "experienced gender specialist" (experienced = evidence of relevant training and at least two years’ experience working in a specialised gender dysphoria practice) without having to consult with their clinical commissioning group.
GPs do not need to refer for a prior psychiatric assessment before referral to a GIC, but may do so while the patient is waiting if they are experiencing distress from waiting times. In line with advice from the Royal College of Psychiatrists, they can also prescribe the lowest acceptable dose of HRT after consulting with an experienced gender specialist should the patient be self medicating with hormones or at significant risk of self harm or suicide. All treatment must be conducted in compliance with the Health and Social Care Act 2012 regulations.
Referral to a Gender Identity Clinic is subject to monitoring by the Care Quality Comission, which ensures the system in place for the management of and reduction in the Gender Identity Development Service (GIDs) patient waiting list is effective. NHS England in collaboration with the Department for Health and Social Care have increased yearly funding by £6 million to GICs to achieve this goal and is also running pilot schemes in London, Manchester, Cheshire and Merseyside to trial delivery of some services in primary healthcare settings to be assessed after 3 years, has obtained additional surgical providers to increase Yeet The Teet capacity and has established the UKs first accredited post-graduate training credential in gender medicine, delivered by the Royal College of Physicians and University of London with the aim to increase the professional workforce.
In like with the World Professional Association for Transgender Health guidelines, HRT may commence if the patient is of the age of majority, has the ability to provide informed consent, has demonstrated to the GIC a persistent and well documented history of gender dysphoria and does not have contraindicated health issues (e.g. venous thromboembolic disease, hypertriglyceridaemia, cardiovascular disease, hypertension, hyperprolactinaemia or prolactinoma). With a patient with severe mental illness that impairs reality testing (psychotic episodes, bipolar disorder, dissociative identity disorder, borderline personality disorder), an effort must be made to improve these conditions with psychotropic medications and/or psychotherapy before hormones are contemplated. Surgical interventions may proceed after 12 months of social and hormonal transition. Further surgical interventions like FFS or body contouring are considered purely aesthetic and therefore not covered by NICE on the basis of lack of evidence of beneficial health outcomes, except in exceptional circumstances with well documented distress.
The point of all this sperging is that Ollie not only is claiming that the NHS should do what he wants when he wants it, but they're actually behaving illegally. This is the opposite. Not only are they meeting all their legal obligations, it would actually be illegal for the NHS to run roughshod over recommendations from these other professional bodies, and would be illegal for NICE to disregard its requirement for evidence based treatment from professional bodies, as it must be demonstrably shown that efforts have been taken to use public funds in a manner that is the most cost effective (and boutique on demand gender surgery is not cost effective if this means less oncologists, for example).
Of course, Ollie's parents could have told him that, but this isn't something he'd listen to. He might have a point about waiting times breaching the NHS Constitution, but he's not complaining about that, he wants nobody to question a trans person demanding hormones and surgery RIGHT NOW.
If he does read this thread then perhaps instead of protesting NHS England who are pretty hands tied, he could protest the Gender Identity Research & Education Society - a society of largely transgender people - who ensured the UK’s Good Practice Guidelines were updated in line with WPATH's guidelines. Those nasty nasty transmedicalists are why you have to get referrals from psychologists at a gender clinic, Ollie!

The 'Low Skill work' talking point has been going around recently and its pretty obnoxious because the idea of low skill work is not meant to be a value judgement that people who engage in that type of work are somehow lesser, its meant to be a materialist understanding of the nature of labour divisions that effect the majority of the working class.
Ironically, the people who are making a big song and dance about how you shouldn't say anybody is engaging in low skill labour are being anti-marxist because Marx was very aware of the importance and ramifications of low skilled labour in his theories, and didn't avoid describing it as such. One of the big things that Marx talked about is the way in which technological advances have continually reduced the overall need for high skilled labour since things that previously took a lot of time and experience to perfect, for example weaving, could instead be offloaded onto an industrial machine where the skill requirement of the individual worker was hugely reduced. In effect the workers themselves were pushed towards behaving in a more machine like manner where they were often doing repetitive and simple tasks in a factory where the aggregate of these tasks could produce something far more complex than the workers would have been capable of doing individually without a huge amount of training and experience, this reached its peak with the development of the assembly line in the early 20th century where extremely complex products like cars could be produced by people who would have absolutely no hope of making a car on their own as an individual, but can as part of the assembly process with many other workers doing relatively simple tasks.
Marx pointed out that the overall result of this was that capitalist production could result in considerable amounts of innovation and huge increases in overall production compared to older methods, but at the same time it was highly likely to reduce the economic power and leverage of the average worker, and could kick skilled workers down the ladder since their particular abilities were no longer difficult to reproduce and in high demand, reducing their ability to do things like demand fair recompense for their labour, instead the balance of power was shifted in favour of the owners of capital like property, machines, land, factories and other things that made for industrial society who reaped far larger returns because of their control of these things than the workers they employed, who, because they partook in unskilled work, were highly expendable and could be replaced easily, seriously damaging their negotiating power.
The last bit is crucial, its why a proper Marxist would scoff at this notion of declaring everybody high skilled workers just to make them feel good. The term is meant to be an objective recognition of reality, if you can be let go and replaced with somebody else with ease by your employer because your job has a large pool of people to draw upon and just about anybody can be easily trained up to occupy your role you are unskilled compared to a job where the exact opposite is the case, it does not matter how you dress it up.
