I'm not so sure it was a miscarriage, it could have been Ralph pressuring her into getting an abortion like he tried to with Faith, May would/could still be affected about it and sad.
There are multiple reasons why it fits:

• The whole point of getting her pregnant was to replace Xander and spite the Vickers and it failed
• Ralph was visibly disappointed when he found out it was a girl on stream
• The only reason they posted pictures of the bump during Christmas was because it was pointed out how they were carefully avoiding any pictures where May's pregnancy was visible. it was fake already by that point
• Ralph was attacking Warski very hard with "you killed your kid". Ralph usually projects his fears/what he hates about himself onto his enemies
• Ralph doesn't really want kids beyond being able to "brag" about being a father
• Ralph tried to get Faith to abort Xander

Of course, it could have just been a miscarriage, but I see getting an abortion while continuing to pretend she is pregnant to avoid criticism more as Ralph's style.
Either way, I am more convinced than ever that there is no pregnancy, with:

• The bizarre baby bump that grows way too much in a short time, disappears, comes back and then gone again.
• Them only posting pictures once it was pointed out how weird it was that they were avoiding it.
• Ralph never mentioning anything about the pregnancy until this thread points out they haven't prepared at all for a kid, and like clockwork the next day he mentions the things that were mentioned in the thread
• Comparing this supposed pregnancy with Faith's where Ralph shared every detail he could when he had access to her
• Ralph saying that they're not going to show pictures? (hadn't heard this one) he even leaked Xander on /cow/ there is no way he wouldn't post pictures to "own" someone
• May showing no sign of a pregnancy (mentally or any other normal way) running around doing errands for Ralph. Could be just her low self esteem but still

Anyway, I've said it before, but with these people the most retarded explanation is probably correct, so faking a pregnancy would fit both these idiots.

If she isn’t pregnant now, then I’d bet she never was pregnant. I don’t see why it’s necessary to throw in the second trimester abortion/miscarriage element to the story. (Maybe a very early miscarriage soon after the pee stick test, that’s pretty common in first pregnancies.). I don’t think her pine nuts allergy caused a second term miscarriage or spontaneous abortion or whatever. I think they’d have to induce labor if it died in utero at that stage, she’s not going to just pass it, or have a D&C at 6 months to get it out. It’s a traumatic thing to go through, and if Ralph skipped off to Vegas and then Portugal right afterwards, and she’s supposed to pretend she’s all happy and pregnant, she’s more batshit than I thought.
