I noticed this and was trying to figure out if it was a scar or a line on her back from something else. She's never mentioned spinal surgery to the best of my knowledge. As much as she's shared about all of her other medical issues and procedures, I'm surprised that we haven't heard about something that would leave a scar. Unless there is another Kiwi that remembers something that I'm just not.
I think it’s a distortion of the skin from the excessive fat on each side of her body. The shape of the mark is too well defined and wide to be a scar. Sort of like the Linea Alba on the abdomen.

In fact, it is one of the most toxic forms of abuse according to Instagram and Facebook. Rejecting is also psychological abuse. So by not wanting her to hurple over there he is further tormenting her.
