If she started at an urgent care, they could have done an x-ray and seen signs of a bowel obstruction. They'd transfer her for more workup then, because that's too acute for an urgent care to deal with.

An x-ray is part of the standard initial investigation of an acute abdomen, but it needs follow-up to determine if it's a partial or a full obstruction, where the obstruction is, what's causing it.
