There's a positive note to this though.

He won't be reproducing. The only chance of this mental illness being passed on are by the sister or possible other siblings having a child that were mentioned earlier or mom having some super late in life baby or their offspring and so on and so forth.

Back in ye olde days, castration was essentially the ultimate punishment short of death. If I was subjected to castration, I'd probably kill myself. Like, losing an arm or a leg is one thing, I could find a way to live a life minus those. But I lose my dick? Just fucking shoot me. End it please.
