Fair enough. I also think it's completely retarded for this situation to escalate to this point. Ralph clearly has severe Cushings. That fat pads (Buffalo Hump) on his back and face leading to a blended chin/neck are a dead giveaway.

The larger question here is cause. While standard Cushings is normally a pituitary issue caused by cortisol imbalances due to stress, a pseudo-Cushings is directly linked to alcohol abuse:

It seems to me a lot of this could be linked to the worsening of Cushing's which is building off Ralph's known apnea, high blood pressure, and highly likely diabetes. The alcohol is really killing him mind, body and soul at this point.

I made a post talking about this in another thread. But yeah, hypercortisolism (linked to CS) could be caused by hepatic dysfunction. Meaning Ralph could be on the onset of more serious complications related to hepatic disease.

I see many people speculating about untreated diabetes and shit but I don't think that is, or will be his main problem anytime soon. If it was he'd had developed acute complications by now. Generally and ironically, lardasses with untreated diabetes drop a lot of weight for no reason at all before becoming slaves to insulin, so I don't think that's his problem right now; even if his insulin resistance tries to get the better of him, It's relatively easy to control it. His shitty lifestyle would make things a bit hard, but just by taking insulin he would be able to control his diabetes until his late sixties. (Maybe losing a limb or two). Another organ is due to give up waaaaaaaaay before that happens.

His visceral fat alone could be screwing with his liver, adding alcohol on top of that is just asking for trouble.

He is just too fat, he can't stop drinking and he loves popping pills. After the Portugal beatdown he was back in America, probably under prescribed pain killers, drunk. That's very telling, Xannyberries are not that hepatotoxic but most painkillers sure are. I don't know what other kinds of pills he abuses but should they be hepatotoxic he's right on schedule to develop liver disease complications very soon. You can't control that long term without putting the bottle down right now (which he won't), unlike type 2 diabetes.

So an overworked liver due to fat, alcohol, excess cortisol and fucking hepatotoxic medication can yeet itself into a giant scar faster than you might think.

I bet his liver is already enlarged and might also present high portal vein pressure. That would very nicely explain the potential Alcohol-induced pseudo-Cushing's and that's really bad. He is not even 40.

Another damning clue that Ralph has developed Cushings (likely through severe drinking) is by comparing photos of his younger self.

Look how fuller he looks here. His upper body is more rounded, as opposed to everything just hanging down because he has lost all muscular support. His forearms were bigger, more general definition. I'd have to imagine he had big, fat-man calves earlier in life too.

He looked like a normal fatty back then. Today he is in a separate category from obese; he's a freak show. The pills have turned him into Tetsuo Shima.

Lol interesting enough the cake says 20th birthday. Literally his last year before his deep dive into Cushing's.

It turns out that corn is almost as bad for you as meth and just as hard on the teeth. Over-consumption can result in Vitamin B3 deficiency, which in turn can lead to diarrhoea and verbal diarrhoea (aka dementia). It can ramp-up the blood sugar, making it unsuitable for diabetics. Bloating, flatulence and increased weight are also common, as is the aforementioned tooth decay.

If Ralph does end up going to jail, or passing away from a mysterious heart attack, road accident or drug overdose, then the finger of suspicion must surely point in the direction of 'big energy.'

It's really a race to the bottom. Who's gonna cross the finish line first? A diabetic coma due in large part to the amount of alcohol he guzzles. A subdural hematoma the next time someone hauls off and clocks him. Which is another condition that alcohol abuse increases the chance of. Stress induced heart attack from seething too much. Possibly an aneurism during one of his drunken hollering sessions. Hell, at this point there's no telling if there's any latent STDs floating around or an artery that's ready to clog. Dr. House couldn't diagnose Wiggerbearpig without 15 additional white boards to write on. Ethan's rampant drug use and alcoholism on top of any underlying genetic conditions..... honestly Ralph already has one foot in the grave.
