As for what's happening in the picture, the scrotum was opened up, the testicles removed, i.e. an orchiectomy, the remaining scrotum was reshaped into a (very bad) approximation of the "labia major", the penis was "inverted" and glans transformed into a "clitoris", and a neovagina was created which we all know how it'll go, rest is history.
The tubing is necessary as it'll bleed for days, it's common in other surgeries too.
I cannot go into more detail as the picture makes me depressive, and I want to eat too. We all have our limits, cutting off healthy organs in psychiatric patients that should be in the looney bin is mine.

Another fine piece of work by humanitarian doctor shlomo shekelstein, trully there was no other way to prevent this suicide.
At one point one has to consider, is living like this (a permenant medical patience with a permenantly open gushing wound that smells like shit and gets infected every other day while leaking foul smells and fluids, unable to have kids or experience orgasms or pleasure) really better than being dead?
