In Germany you have to types of health insurance: the general one and the private one.

Basically everyone who has a job, student, child or who lives from social welfare has access to general health insurance.

If you call an ambulance the costs are covered mostly by the insurace and the patient pays around 10€ only to contribute (the whole costs are something between 300-600€).
If you get caught calling the ambulance fraudulantly they will slap the whole costs onto you.

But usually the hospitals are rather generous and won't snitch on you if you call an ambulance when it wasn't really necessary. You have to pull off something really stupid to pay for the whole ride. Even when you get completely wasted and hurt yourself while being drunk as fuck and call the ambulance they won't charge the full price.
