I mean, viewers (cunts) report the stream if they recognize someone who was previously banned. And yeah, streams reviewed by a human modteam

Twitch likes to shoot first and ask questions later, or not ask at all. Only when you reach out to them, they'll (probably) unban you if the appearance of banned person was purely coincidental. But what the chance of such event happening in the first place?)
 
As far as I know they do. If streamer's channel was banned on twitch it also extends to appearances on other channels, so the another host gets banned as well (not sure how hard though if it's first strike)
essentially the way it works for twitch at least is that if a streamer gets banned they are not allowed to even show up in the background of someone elses stream. so lets say keffals was doing an irl stream and destiny just walked past and said hi. if that was being streamed on twitch keffals would be banned.

HOWEVER for some reason this does not seem to apply to clips. as can be seen from everyone on twitch watching destiny covering all the recent twitch drama. nobody as far as i am aware has been banned for that even though some people are just directly watching it with his face on screen and everything. its really weird but twitch makes zero sense when it comes to who it bans and how much they care about a banned person showing up or any of that.
