Therapy is the same for them (and also a big money industry), they don't want to change because they usually don't think anything truly is wrong with them personally, they want a quick fix (also the name of a book by Kiwi Farms member singal69) that will cause all their problems (caused by others) to fade away. Latching onto communism is the same kind of thing. Just look at how Keffals jumps from thing to thing with total short-term focus and a determination that it's meaningful and will lead to him finally getting the success he's entitled to just by being himself. Communism, streaming, ratioing people, DropKiwiFarms, TwitchCon, drawing a line on his moobs, apparently now detransitioners, etc. After none of this works he just moves onto the next thing and mostly forgets about the prior stuff unless there's some other reason in his "new" interest to be attached to it.

I will confess I shouldn't get upset at cows, but I think getting MATI over pedophiles and rapists shielding themselves from criticism using a very serious mental disorder is fucking disgusting. The solution is to walk away and hope they get prosecuted someday.
And yeah, dysphoria is a mental problem, but the only known solution is HRT.
Should there be research into other solutions? Absolutely. But therapy and all other medications have shown no results, or had side effects that outweighed the positives.
