Hey guys, I just thought of something: why don't they just use breast tissue for phallo?

I know none of the doctors were able to think of this, because I am much smarter than them. I only ever have original thoughts, so I also know that nobody else was ever able to form this idea in their brains.

Here's a thought that came into my mind while reading this: we know that there's a gross over representation of autists in trans population rn. Autistic people are known to stuggle with sensory processing dysfunctions, including oversensitivity to touch (tactile stimulation). Could it be that part of the reason so many of the troons are driven to these surgeries and show an apparent indifference towards common complications such as loss of sensation is because they perceive tactile sensation as painful/uncomfortable and would rather lose their ability to feel touch in certain parts of their body?

Like I'm not saying that we could've saved these pooner spergs with sensory integrative therapy but could we at least try with the younger ones?

Here’s a new and interesting article regarding a novel technique of using a gracilis mucle to form a neopenis. This article was published back in January so I hope I’m not late posting about this.

Some backround: Apparently in Finland there’s a unique method of phalloplasty called “gracilisphalloplasty” that was invented back in 2010 by two female butchers Maija Kolehmainen and Sinikka Suominen who both work in HUS/HUCH (Helsinki University Central Hospital). Interestingly, even though this method is claimed to be the most commonly used method in Finland (according to an article published in 2015) there’s practically no information about it available whatsoever.

The only information I’ve managed to find is a Finnish article about transgender surgeries in Finland written by Suominen & Kolehmainen. Here’s what is has to say according to google translate:

“Neophallos is not a penis, meaning that even in a best-case scenario the reconstruction results are unimpressive in terms of appearance, sensation and function. The erectile tissue surrounding the biological man’s urethra contributes to the draining of the urethra, but in a neourethra build with skin and colonized by bacteria there remains urine, causing dripping and odour disturbance. There is always permanent scarring in the operating areas, depending on the type of tissue, possible wound healing problems and tightening. Testicles often seems to be situated too much between the legs, due to the lack of musculus cremaster that controls the height of testicles with biological men. Erogenous sensation is located in the area of the clitoris left under neophallus but may have deteriorated with scarring. The benefit of functional phalloplasty would appear to be a lower rate of tissue death and urinary fistulae, as well as inconspicuous donor sites. Because of the scarring of skin grafts, the obstruction of the urethra may need to be corrected later. Some patients have reported that muscle movement can create a so-called paradoxical erection and enable intercourse.”

There was also one literary review about trangender surgeries in Finland but it unfortunately used the previous article as its only source for gracilisphalloplasty – although the writer seems to be a tad more optimistic compared to the source material…

The skin graft comes on top the muscles, making the whole look natural. With this technique, the voluntary movement of neophallus is possible, which may also enable erections and intercourse. The benefit is also inconspicuous tissue collection sites and tissue death and a lower risk of urinary fistulae.

Does anyone here know where to find experiences of people who have gotten phallo in Finland (blogs etc.)? Or specifically the gracilis technique, Finland or anywhere else? I'm very interested in that one especially, but I haven't been able to find any information through google on people's satisfaction etc on it once it's healed. Only the technical info on how it's done by the surgeons here, and one blog where the person had a different kind of phalloplasty and briefly mentions the gracilis technique.

PS. Is it a surgery that's only done in Finland? Or does it perhaps have a different name in english? I've tried googling in english as well, but to no luck

For a long time this was everything I could find about this miracle surgery that gives you erections and perfect sensation etc. But then came an article published in January 2023. This article is not written by butchers Kolehmainen and Suominen and the research was done on male patients suffering from micro penises not on trans patients. However, it seems to be somewhat similar method and gives more information than the stingy Finns have done in past seven years.

Background: Phalloplasty is a challenging procedure in the field of repair and reconstruction. Traditional skin flap methods with prostheses or autologous tissue implantation are still common; however, with this method the reconstructed phallus is static and has a high risk of complications.

Aim: We aimed to use novel methods for phalloplasty to mimic the normal structure of the phallus while maintaining the patient’s ability to obtain sufficient hardness and subjective controllability.

Methods: The neophallus comprised the bilateral pedicled neurovascular gracilis muscle, which provided neophallus volitional control; the pedicled fascia lata, which mimicked the tunica albuginea; the pedicled anterolateral thigh flap, which was used to reconstruct the neourethra and neoglans; and the thin split-thickness skin grafted on the surface of the fascia lata. The urethral anastomosis was performed simultaneously. The appearance and function of the neophallus, as well as patient satisfaction, were evaluated postoperatively.

Outcomes: The biomimetic dynamic phalloplasty using the bilateral pedicled gracilis muscle combined with the fascia lata was successfully performed in 4 patients.

Results: During follow-up at 10-21 months after the procedure, all 4 patients could complete sexual intercourse and were satisfied with the appearance and self-controllability of the neophallus. The size of the neophallus stabilized within 6 months. The tactile, deep touch sensation, and pain sensations of the neophallus partially recovered. All of the patients could stand to urinate.

Clinical Implications: We used the gracilis muscle combined with the fascia lata to mimic the basic structure of the corpus cavernosum and tunica albuginea and successfully reconstructed the biomimetic dynamic neophallus.

Strengths and Limitations: This is to our knowledge the first report of biomimetic dynamic phalloplasty using the bilateral pedicled gracilis muscle combined with the fascia lata to mimic the complex structure of the phallus and enable the achievement of erection. However, due to the small number of patients included in this study, the data have no statistical significance. More cases are needed to obtain conclusive data and examine the long-term clinical effects of this procedure.

Conclusion: Using novel methods for phalloplasty, we successfully maintained the patient’s ability to obtain sufficient hardness and subjective controllability without the use of implants, and the preliminary clinical results are encouraging.

When the neophallus was squeezed or pulled 2 days after surgery, all patients had deep sensation. One case had partial skin grafts with poor healing, which was completely healed by conservative dressing change. In 1 case a urinary fistula occurred in the urethral anastomosis 1 month after the operation and healed well after the second revision operation. During follow-up at 10-21 months after surgery, all patients could perform sexual intercourse and were very satisfied with the appearance and self-controllability of the neophallus. Six months after the procedure, all patients could contract and swing the neophallus autonomously according to their intention and achieve sufficient hardness. During the first 3 months, the length and circumference of the neophallus demonstrated partial atrophy but stabilized within 6 months. The tactile, mechanical stress, and pain sensations of the neophallus partially recovered within 9 months. All patients could stand to urinate, and the urethral continuity and patency were good

AND it came with some photos. Obviously, these are male patients, but the technique seems similar to the Finnish surgery.
