Can anyone like @Aunt Carol or @Manul Otocolobus comment on this vitals as a whole? I don't normally think about KF much when I'm not actually on the site, but Dave's readings perplexed me and my medical training pretty much starts and ends with keeping a body breathing and plugging leaks until someone with real book learnin' shows up. The numbers he's so proud of still look worrying to me.

I know BP and pulse don't scale linearly, but wouldn't you expect to see higher readings at 120+bpm? Dave is on the low end of normal as I understand it at 105/65. Wouldn't a normal resting heartrate drop that into low BP territory? I'm just thinking of basic fluid dynamics. Does his heart work so poorly that the pump needs to spin nearly twice as fast as it were? I guess with someone as fat and sedentary as Dave that it'd be more like a +30-40% increase. Lose some weight fatty. Give your heart, back, and joints a break.

I also love we have no proof of the sky high blood pressure his father caused, but we do have proof of some kind of heart condition that he refuses to try to alleviate by changing his slovenly lifestyle. Wait, I forgot he needed to get 300% of his daily sodium and 200% of his daily fat from microwave meals because he didn't have the spices to put on an apple.

I don't know that specific monitor, but I'd be skeptical of that heart rate, because the oxygen reading is lit up too. This means that very likely the heart rate is coming from the fingertip pleth, same as the oxygen saturation.

With a self-reported irregular heartbeat, I wouldn't immediately trust the machine;. It's counting a few seconds of fingertip readings and then just multiplying. If someone has atrial fibrillation, the values can fluctuate wildly and seem scary, but it's just because the machine went from multiplying beats 1 2 3 to multiplying beats 3 4 5. This can get really weird-looking if someone's simultaneously on two different monitors and they calculate differently

Dave is on a handful of meds, but I forgot if he truly has AFib or it just makes the most sense for the things he takes and the things he claims. (I remember he was pity-farming about "heart surgery" and it turned out to be that he's putting off a catheter ablation.)

Anyway, my Tier 2 tech support answer is to confirm your theory. Dave isn't a trauma victim (shh) but has a dysfunctional heart and/or terrible physical fitness so his heart is working harder in order to maintain a normotensive BP, and he might have just engaged in rigorous activity such as walking from the waiting room to the exam room. We should still check an apical pulse to confirm this isn't mechanical error, so please do the needful.

AFib was inferred based on the meds he takes. I agree with AC. That type of machine isn't particularly reliable when it comes to measuring a consistent heart rate.

But, for fun, let's consider the numbers are real. Those numbers, with the meds he's on, would represent a problem. He's on a calcium channel blocker for rate control, but also Prazosin for his PTSD, which also happens to be an anti-hypertensive. If those numbers are accurate, his meds need to be worked on. His rate is unacceptable, being stage one tachycardia, and particularly so for someone on rate control meds. His BP is borderline low. It's possible he could be tachycardic due to the low BP, and his body is defeating the rate control meds to try and elevate the BP. In this situation he needs to drop the prazosin since its provoking hypotension, and see if everything improves. If the BP improves, but the rate is still tachy, they'd need to try and adjust his calcium channel blocker to get his rate below 100 without impacting his BP by much. Given the type of calcium channel blocker, it won't act on the BP through any vascular mechanisms, but it will drop relative to heart rate. Peripheral vasoconstriction would allow for rate control and a reasonable BP. But, some people just don't respond how you would want, so if you're not careful you could bring the rate down how you want and this could tank their BP. For someone that is borderline to start with you will have your patient showing up to the ED as a syncopal episode of unknown origin and a very annoyed ED doc (syncopal/syncope is a sudden loss of consciousness, in this case it would be from hypotension. The mechanism is caused by insufficient blood flow to the brain to maintain consciousness, but it is meant to be corrective. Once one is lying flat, blood flow to the brain increases since the blood doesn't have to pump against the force of gravity. Once blood flow increases, consciousness is restored. So, the syncopal response is both corrective, and protective, since you can't very well run a brain without enough oxygen, since that would lead to an energy deficit, and that is a serious problem for brain cells).

Assuming the numbers are accurate, I'd adjust them to get BP up slightly (if possible) and get HR below 100. After that I'd tell him to stop being a lazy, fat fuck and lose some fucking weight and move his ass.
