How/why does this even happen? I know a guy who started balding in highschool, but even he didn't reach this level by 26. And this poor girl probably started taking HRT a few years ago, so this must've happened very quickly.

I had a guy go bald in highschool. Another had so much white in his hair it turned silver on the sides. The bald guy was a really heavy drinker and the other guy a druggy. I think the balding is almost entirely genetic. I got horse shoe hairline in my teens/early twenties. Hasnt changed all that much over the years luckily. I have a little scar right at the hairline and it helps me keep track of it

It can be hormone dosage to body mass ratio. Or it can be from the simple fact that synthesizing something and taking it externally is going to have different effect than a body naturally producing it. It's not unique to FTMs freebasing T, I'm not going to claim it's super common but I knew a guy in high school with possibly the worst genetics for baldness I've ever seen. He was norwooding at 13 and completely bald by 16 as were his two brothers.
