They were sort of an exception, though. They allowed adult males who'd already had families to just lop their shit off to gain prestigious jobs. Being a defenseless child wasn't as strict a requirement in the Chinese court as it was for the Italians.

It's interesting to think about the historical reasons that people practiced genital mutilation and compare it to today's situation. Castrating children was done to give them a chance at social mobility or to save their 'souls' (Skoptsy, African FGM). In other words, there was significant social and economic incentive to do this to kids. There are a lot of trans advocates that find it beyond their imagination to picture any reason why families would want this done to their kids today. But lots of Italian and Chinese families ripped their kids' dicks off just for a chance at a shot out of poverty. If you're being told by advocates that your kid WILL die without castration, wouldn't that be pressure enough?
