Barbers can be pretty weird and chatty. They also have you as a captive audience.

Possible, but would still have some holes.
Pretty much any leukemia or lymphoma that Nick would have developed at his young age would have the option of chemotherapy.
We really haven't seen Nick disappear for long enough to have undergone such a course of therapy. Similarly unlikely that he would've been able to drink all the way through it. Or travel as much.
I suppose it could account for some of his missed shows or his daytime disappearances or constant exhaustion or deteriorating physical appearance.
There's also the factor of traveling outside the US and health insurance. Standard travel insurance won't cover preexisting conditions if you get sick out at Hedonism II.
I had a terminal family member who loved to travel. They ended up buying some sort of medevac airlift insurance plan that would pick them up wherever they were in the world to bring them back to maple-flavoured glorious socialist paradise for treatment if needed.
It's possible that Nick was told that he had some leukemia or lymphoma precursor with a natural history timeline, with some blast cells on routine bloodwork or a Myelodysplastic Syndrome.
But still, at his age, you would presume some chemotherapy would be involved.
Who knows though. Norm Macdonald was able to hide it.
