Results:
Overall, 869 and 357 patients were identified undergoing vaginoplasty and phalloplasty with 193 (22.2%) and 74 (20.7%) having at least one psychiatric encounter (Table 1). Although the overall proportion of those experiencing a psychiatric encounter was similar between the vaginoplasty and phalloplasty groups, suicide attempts were more common in the vaginoplasty group (4.4% vs. 1.7%, p=0.033). The rate of a psychiatric encounter occurring after surgery if an episode prior surgery occurred was 33.9% and 26.5% for the vaginoplasty and phalloplasty groups. The overall rates of suicide attempts doubled (3.3 vs 1.5%, p=0.017) after vaginoplasty (effect not observed after phalloplasty).

Conclusions:
Rates of psychiatric emergencies are high both before and after gender affirming surgery. Although both the phalloplasty and vaginoplasty patients have similar overall rates of psychiatric encounters, suicide attempts are more common in the later. In fact, our observed rate of suicide attempts in the phalloplasty group is actually similar to the general population, while the vaginoplasty group's rate is more than double that of the general population. Patients undergoing GAS with a history of prior psychiatric emergences or feminizing transition are at higher risk and should be counseled appropriately.

I find the disparity between suicide attempt increases between the troons and pooners particularly fascinating. The rot dog has always been the more horrific of the two surgeries in my opinion --an accolade about on par with winning a Most Handsome Pedophile competition, but still-- not least of all because it comes with a bonus horror on your arm/thigh, looking like you've survived a flesh-eating bacteria.

I guess it highlights the differing motivations between the two groups: pooners don't care how ridiculous they look or if their cock actually works, as long as they don't have to be a woman anymore; troon's sole motivation is chasing the coom dragon, and once they realise castrating themselves removes their sex drive there's no reason to live.

I wonder about this. It uses attempts as a metric, actual completed suicides you expect to see much higher in males, they tend not to do the cry for help stuff and just go straight to actual death. BUT I wonder if the lower pooner rate is linked to the multi step and multi year process of a rot dog vs a mystery hole. Installing an extra hole is a one and done (discounting revisions to fix fuckups) whereas making the sausage often takes a long time. These girls are ‘in process’ for years and so they’re still chasing the dragon. The men have realised that’s it and it doesn’t get better.
I will be interested to see the attempt rates 10 years post. And also the rates of UTIs and kidney failure in the women.
