As a therapist, I have never seen emotional injury as a factor and that's fucking mental (to me) to be in there. Therapy is sometimes going to hurt, we don't grow if we're not uncomfortable, challenge is what grows us as people. I've had to report things before but it's only crisis level stuff, where they are likely to go out and hurt themselves or another at some point.
