The liver is a surprisingly resilient organ. When an alcoholic develops liver problems, most of the time there are other exacerbating factors - e.g. an extremely shitty diet, other substance use/abuse (OTC painkillers are especially bad). He's only 40 and according to Aaron only started using within the past couple years.
Assuming they don't die from an OD, plenty of people are able to keep up heavy drug use for decades. Lots of alcoholics make it to their 50s and sometimes even longer. Their quality of life might be shit for most of if not the entire time, but a lot of people's bodies are actually very resilient and take a long ass time to completely give up on them.
