At worst one could argue that he is not helping her to get better or even make it worse by just going along with her whims. Antipsychotics can result in rapid, difficult-to-control weight gain too. If the documents in the Kat Speculation thread are real, she was almost institutionalized by court order. This is not something done to help someone who's getting by but struggling, it's reserved for people who are so mentally ill they are a danger to themselves or others (so suicidal tendencies or extreme mental illness). 2017/2018 Kat could have been (relatively) stable without medication, then something snapped post move to Renton, she got put back on psych meds, and ballooned up because of those.

I don't think she was ever particularly active. IIRC, the only time friends was mentioned is during her Halloween stream experience, and that was retconned like 4 or 5 different times. We also know Phil was lying a lot about her being "at work" during the week, but then we found out she was only working about 20 hours a week. I think that was his justification for changing his day off, it was the one day a week Khet was available. The gym stuff could be another lie, but even if we're being extremely generous and assuming that some of the gym trips were true, I don't think Khet is unique in trying to work out more, but ultimately fizzling out after a few sessions. Hell, assuming it was true (which is already a stretch) I'll give her the faintest bit of credit for keeping at it longer than most new years-esque attempts.
Khet didn't morph into her slob self overnight, it just feels like it because there was a long gap between appearances. She's fat, there's no doubt about that, and it definitely isn't healthy, but she isn't like...deathly obese fat. All that being said, I do think Phil encourages her laziness/slob behavior, but only in the sense that living with him is likely the first time Khet could fully indulge. As far as we know, she wasn't really fit or socially active in the past, and I'm sure if she had the money back then, she'd be living like she is now.
