Beyond her inability to save herself it is too late for chinz, in both situations she is absolutely fucked. Canada, like other western health care systems, will make you wait for anything elective and other not important medical issues, but I am pretty certain that she would be seen very quickly with all her medical conditions. At issues, will Chantal be compliant with anything her medical consultants tell her? Will she go to all her appointments? How to solve her conditions? Lose 2-300 pounds and get on a diabetic friendly diet. Will Chantal do this?

ETA now roughly an hour has passed since her insulin injection. She's been live for 2,5 hours.
She takes her BS again and now is frustrated, that it is even higher while on insulin.

She says she's frustrated and tired of keeping track of all this shit.
Also says, that she doesn't know her weight after her last 15kg loss (didn't she tell us then, it was 17?).

Yes you are kind of correct. The one thing you are not accounting for is scale of the dosage where your example is for an averaged sized person. For instance, blood sugar levels are just that...how much sugar is in the blood. But with Chantal she has a lot more blood than a 150 pound person, therefore if she had a blood sugar level the same as someone who was 150 pounds, and you somehow extracted all the sugare out of each of them and weight them, she'd have overall more sugar.

So lets do some math. we'll jump over to kilograms cus..ug. fine. Lets look at average blood volume of a patient. The formula for a woman is: (men have more blood)

Estimated blood volume = weight (kg) * 65mL/kg

The high end healthy weight for a woman Chantals height is about 60kg (133 pounds) * 65ml/kg = 3,900 mL (or 3.9 liters) of blood
Chantals claimed weight is 158 kg (350 pounds) * 65ml/kg = 10,270 mL (or 10.37 liters) of blood

Ok. Now we have that info. Now lets look at the blood sugar metrick. If Chantal claimed a blood sugar of 10mmol/L, that would mean for every liter of blood, she had 10mmol's of glucose floating around.

The healthy yet chubby girl at 60kg = 39 mmol of actual blood glucose
Chantal at 158kg = 103.7 mmol of actual blood glucose

As you can see for the chubby girl to get to a normal 5mmol, she just has to "get rid of" a mere 19.5 mmols of glucose where Chantal has to get rid of 51.5 mmols of glucose.

Now with your simple example formula, which again is probably just an average sized person, 8 units would likely just bring her blood glucose down 8-10 mmol/L. Since we know Chantals "normal" is in the high teens to 20s, this would bring her down safely into the 8's or 9's without her dropping below the ideal 5 without her doing too much math.

Not to go too deep into insulin function, but the more sugar=more insulin to counter act it. Chantal would need twice the dose to counteract the same blood sugar level as a smaller person.

What we dont know is if Chantal is talking about a maintenance dose, a mealtime dose, or a correction dose. My examples above are essentially correction dose referred. But it also shows just how much fucking food she eats.

Also, is she taking fast acting? Regular insulin? what are they giving her?

eta: This probably is why they wanted Chantal to go to diabetic education classes. They want her to give herself insulin shots...which requires that she closely monitors her BG levels, records them, and know how to math how many cc's of insulin are required. Impossible. She gonna die.

@lol cow supreme according to Chantal, they gave her a long acting insulin, in a pen, to be taken at night. This in concert with oral metformin. The box of syringes would indicate that's not the whole truth.

Though for sure when you actually have diabetes, because your body can no longer process sugars any longer, we have to eat them with care but they are not the cause of insulin resistance.
The dumb bitch is basically standing in the driveway, lecturing gawkers about what doesn't cause house fires, while her house is fully engulfed in flames. Un-fucking-real.

There are real diabetes plans that consist mostly of carbs (Mastering Diabetes and Starch Solution being two popular examples). But they only work if you heavily reduce fat (like 15 grams a day, max). So she isn’t wrong, it’s just not common practice and very very hard to do. And there’s no way she’s going to do it correctly, even if the science kinda backs her up.
I know a T2 diabetic in his late 60s who is in remission on the McDougall/Starch Solution diet.
He's scrawny, with no muscle mass, but his HbA1c is normal, and he's seen a bunch of other health issues clear up (he still has diabetic neuropathy in his feet, because the damage is done).

The diet is fucking grim, however; the only fat he eats is what's present in nuts/seeds and avocadoes, and he doesn't eat much of those. He eats very little fruit, and no added sugars. The bulk of it is potatoes, sweet potatoes, root veg, legumes, and whole grains, with green veg secondary.

There are no dipping sauces, no dressings with oil or sugar, no animal products, and he watches his sodium intake. Personally, I'd rather go full carnivore if it came to that, but he's used to the limitations, and doesn't mind them because he's still alive, and gets to be there for his family.

Chantal? She could never.

She wasn't seen quickly when she was there during the winter, except in the ER. She doesn't have a primary physician (and the wait to get one is long), so she can't get referrals to an endocrinologist or any other specialists. The best she can get in Canada is basically what she's paying for in Kuwait—periodically getting pulled back from the brink at the ER/a private clinic, before getting discharged to repeat the cycle because she's noncompliant and won't stop stuffing her face.

She has never done any of this before; why would she do it now?

Actually, getting back on the Janumet to bring her BG down from the 20s to 9-10 has been the closest she's come to compliance, and she must have been genuinely terrified in order to do that. But she's still eating troughs of carbs with grease and cheese, against all medical/dietary advice, and now that she has insulin she's just going to use it to offset the effects of her binges (which is why she's wanted to be on insulin in the first place).

She likely has a pen that she does indeed use at night. That would be a stable, constant dose of insulin she gets regardless.

The syringes would be for what’s called “sliding scale” insulin. Usually a different type of insulin that is given at certain times (before or after meals.) The dose is based on the glucose reading at the time. It’s a little more finicky than the pen, because you have to draw up the dose after calculating it, but the pen is atypically a pre-metered dose. And multiple pens can get confusing and expensive.
