a) high
b) low

Fat Fuck says his doctor said the hormone was SO high that his doctor was worried he could have additional other cancers that he needs to be tested for immediately. Dr. Honeybadger lets Fatty ramble on and on about how high his hormone level is and that meant his cancer Poly Vera was super for sure due to the high level of that hormone.

Dr. Honeybadger then says, ok well this Poly Vera cancer manifests with an unusually LOW level of that hormone
