If his kidneys are struggling they are likely wanting to preserve his veins in his arm in case he needs to go on dialysis and get a fistula installed. In which case, the usual place to put your IV or draw blood, even in normal exams, is the back of your hand. It can be other places, but if his veins are especially weak (which is likely due to his age) they'll just do his hand. Elizabeth II had that same mark.
