Aside from helping the down-ballot it serves two important PR purposes:
1. For Trump supporters in blue states, the fact that he’ll haul his ass over to their lost-cause locales despite knowing full well that their votes won’t help him win the election shows that Trump cares about them, not just their vote.
2. It projects an aura of confidence and suggests that his internal metrics show he’s doing well enough in “safe” and “battleground” states that he can afford to allocate some money to doing vanity rallies in enemy territory.

I could be woefully inaccurate but I thought keeping the oath was part of maintaining your license to practice medicine. Which of course you would need to maintain your job. If you broke it you lose your license and thus job.
