You're not from this part of society. Rest easy at night knowing that Kathy Rumer is painfully aware that she is lower-upper class and deeply class-insecure at best. Being a doctor in the US isn't the guaranteed money printer of the 80's and 90's. Definitely not if your main selling point is "I take Medicare". For a plastic surgeon of any stripe that may as well be an admission of incompetence. Most plastics won't reimburse at all to Medicaid, SRS being one of a few exceptions, the other one coming to mind is BA for breast cancer patients post mastectomy. Private pay is much more common in plastics. Mostly the best surgeons score well enough to get into plastics fellowships are skilled enough to enjoy that private pay grade.

Gender medicine is poorly regarded among doctors, regardless of what the publicity office in their hospital says. Im incredibly wary about plastic surgeons who preform all operations head to toe like Frau Rumer. Ideally you want specialists, like an otolaryngologist (eye, ear nose & throat) doctor doing the face and another plastic reconstruction doctor doing the body like liposuction, breasts, BBLs etc. Usually this means there will be two doctors in a partnership or just a practice that focuses on one area. Even Dr. Nassif & Dr. Dubrow, from the show Botched have a setup like this. Oral and Maxillofacial surgeons also do the face but they are mainly teeth and jaws.

Theres a couple of surgeon who can do it all, but those are few and far between, and they command high prices. You find them in Beverly Hills or NYC. Dr. Harrison H. Lee comes to mind but after seeing Dylan Mulvaney's botched nose, Im not so sure now.

What she does to these creeps, you couldn't do that to people you didn't despise, to some extent, unless she's just the kind of psychopath that doesn't hate them, per se, she just doesn't see them as human or anything other than a paycheck.
