This is a good question and a perceptive one.
. I think it’s a combination of things that are manifesting in a novel way due to our cultural zeitgeist:
I think it’s a combination of having a background susceptibility (autism, possible other brain issues akin to a sort of prosopagnosia*, dysphoria predisposition) then that meets PORN and mental illnesses like cluster B stuff. That means you get weird paraphilias. Those then get amplified by internet and they have no restrictions because we’ve removed the ability to shame people. The combination is then
Susceptibility + cluster B disorders + /abuse/manipulation of normal sexuality = paraphilia, the paraphilias + amplification - usual social brakes = rampant and novel form of manifestation. That manifestation is troonism and all its offshoots.
People have been susceptible and mentally unwell forvecer. What’s new is this manifestation, and the lack of societal restrictions, which leads to a frankly monstrous manifestation.
Five hundred years ago the girls who are pooners now were holy maidens. Forty years ago they were anorexics. For the men, only the very powerful would be able to indulge such exploits outside of very niche situations like cults. A hundred years ago you’d get people like this killed by their exploits, or banged up in asylums. To even speak of such things would be unheard of. The social pressure to conform and the local nature of society stopped like minds from meeting, mostly. When they did meet, terrible things happened. Now all of those brakes are off.
I honestly believe that what we are seeing is maybe not a new mental illness, but certainly novel in how it manifests

You hear people say they can’t recognise a face as a person as a whole, and the way they get round it is ‘Susan has the big red glasses and long blonde hair.’ They see people as collections of individual traits but cannot recognise their own wife across the room. What’s that got to do with troons? Well you also hear them say they can’t navigate, they seem to see places and pretty much everything as like a collection of ‘smaller stuff’ and be unable to recognise it as a coherent whole. Now think about ‘woman is when bolt ons and a dress.’ They’re seeing they have these individual traits and don’t seem to understand how others don’t see them as a whole woman. There’s an error somewhere in how these people are categorising the world. That’s also not mental illness, it’s the susceptibility part.

You guys do realize that mental illness exists on a spectrum, right?

Even though some might argue that Freud's original and somewhat archaic division of labeling every manifestation of mental illness as either a psychosis or a neurosis is archaic and wrong, there is some utility to it. The psychotics are the most severe cases; they are literally not oriented in physical reality. They have delusions (I'm using the clinical definition here. What year it is, who's president, etc.) and hallucinations (most commonly hearing voices). These people are incapable of looking after themselves and don't have any insight into their own illness. Care needs to be forced on them, and most countries have a legal framework to do it.

A neurotic, on the other hand, is oriented in material reality, and most importantly, their condition is ego-dystonic, aka something they wish to eradicate in themselves (abnormal anxiety response, obsessive thoughts, etc.)

In the borderland (yeah, this is where the term "borderline" comes from, it literally means "borderline psychosis") between a psychosis and a neurosis, we have personality disorders, of which BPD is the most common. I think most troons have BPD traits. Back in the 90's when I studied, we had different labels, but we differentiated clinically between severe borderline (called gränspsykos in Swedish; I don't know the proper English terminology) and less severe borderline (karaktärsneuros in Swedish; I suppose a direct translation would be "character neurosis", ergo a disorder that is more severe than "just" a neurosis but still much closer to a neurosis than a psychosis).

Affective disorders (depression, mania, bipolar) were sort of their own category, and even Freud's contemporaries recognized this. I think Freud did too, but he never solved this problem in his lifetime.

Classic psycho-analytic therapy was tailored to "suit" the neurotic mind. Freud noticed that therapy would not benefit his borderline patients. In fact it often made them worse to the point of attempting suicide.

Anyway, my point is that troons and pooners are generally mentally ill, but most people's idea of mental illness is limited to the concept of psychosis, ergo someone who fails reality testing. Personality disordered people may seem perfectly oriented in a shallow sense, but once they're subjected to stress they resort to the phenomenon called "splitting", which means they categorize everything (themselves, other people, objects, ideas) as either all good or all bad. No nuance or grey area. You can see this with troons when they impulsively ditch their families, jobs, and everything that should matter to pursue "their true self". This raises the question if they can actually consent to any procedures, and if so, what the clinical limitations should be.

While "mental illness" is a quick and handy description, (and truly appropriate in some cases) some fetish based trooning out could be better described as "maladaptations". When the biological imperatives (behaviors which ensure survival of a species: https://archive.ph/abgP8 ) are corrupted or become misaligned due to environmental or whatever circumstances, the result is called a maladaptation. Maladaptations can be carried (and compounded) over generations too. Maladapted parents raise maladapted offspring, etc.

Most people misunderstand Darwinism: it's not the "fittest" that survives, it's the most adaptable. At the time he formulated his theories, modern maladaptations were just getting started. If Darwin were alive today I'm sure he'd include them.

Yes. A perfect storm of tech and progress altering culture. The desire to physically troon out has been building for about 100 years (there were some who fantasized it in the 1920s iirc) but it really wasn't until medical bio tech, personal computer tech, and social progress (the sexual revolution and then idpol) reached a certain point that it had a way to be realized and condoned.

Parting thoughts: Even as an adult, it's difficult to recognize something as maladaptation material and consciously defend yourself against it if it's widely accepted as a progressive social good and "liberating". Or if the State or the Market imposes it on you, with carrots or sticks. We are all creatures of our times. And these times are very complicated and fast moving.

Yup. The massive anxiety that some women get postpartum is this. Women who were anxious and hovered over a baby constantly had a better chance of that baby surviving than ones who left them in the brush and wandered off. So some degree of anxiety around children and safety in general is massively beneficial. In our current society, we seem to have fewer immediate threats to life like predators so this anxiety gets channelled to more abstract things and it become maladaptive. It’s extremely hard to get rid of, simply knowing the above doesn’t help much.

Our environment and society has changed so rapidly - I’m only middle aged and I feel like the world has changed beyond recognition. The generation above me must feel like aliens. We haven’t changed physically so fast and society is sick and it’s driving us, literally, mad.

Troonism is a symptom of a very deep psychological sickness. I believe it’s a spiritual sickness too.

It can be pretty broad. When I typed this post I was thinking about school and puberty specifically. If they were bullied for getting boobs (likely) , or if they started getting catcalled (happens more in dangerous places), they'd start to miss the time they didn't have boobs and were treated nonsexually.

Though there are cis women who are addicted to plastic surgery. They're probably adjacent to the pooners. It could be something genetic, but then the parents could still be to blame.

This is an important point. Just because at face value it seems someone had an easy homelife doesn't mean that couldn't be predated on. It's frighteningly common for this to happen through online and almost everyone has had it happen to them, almost happen to them, or knows someone who it did. At this point allowing your children access to this stuff is neglect. (And yes it is possible to disallow major access 24/7.)

Another important point to make is that child abuse isn't always obvious. It's common for kids born to relatively well off, or even wealthy, parents to be neglected or abused whereas their poorer peers are raised in a better environment. We see pooners and troons mostly showing up overwhelming in wealthier and deviant/neglectful homes. I'd say the internet being used in lieu of irl friends is the biggest sign that a kid is susceptible to trooning out. It isn't just access to the internet. The internet replaces healthy socialization behaviors with their near-age peers.

Hell, just look at how home schooled kids seem positively autistic and incapable of socializing normally. Trooning out seems to be a nurturing failure compounded by autism, social problems, and grooming.

As for why I (not DSM-5) consider transsexualism a disease, I subscribe to the Hybrid Definition of Disease. A Disease is a phenomenon that occur in an individual person that satisfy both criteria (1) It must have a presumed biological origin OR causes a biological disturbance. (2) It must affect the individual adversely. (2) is both objectively and subjectively true. (1) is debatable, but brain studies suggest it is true, and I find it plausible.
