Normal Illya is 18 (just ignore that she looks like a midget at best and a middle schooler at worst), doesn't have a route in the original F/SN (which IIRC also means that she has no sex scenes), and by and large does act like a woman her age.
Prisma Illia is the worst of the spin offs. The only good thing is that it's mostly self contained. It really doesn't affect anything outside it's bubble from a lore perspective. It's a benign tumor. Still cancer, best if you removed it, but it's not spreading (right now anyway).
