I'm no expert on this subject, but the bolded part could be the sign of previous 'roiding. I have heard of some men who use anabolic steroids having their jaws grow so much that the gaps between the teeth (which are a fixed size) become larger. Nick clearly has an unstable self-image, so a past of juicing would not surprise me.
