The fact that it took the day after to hurt and be immobilized just confirm that he is still using heavy doses of medication. His system was knocked out by the substances whatever he is taking now.
I have hurt my ankle 10 years ago, and I swear 20 minutes after the accident the pain was agonizing. It does not take 12 hours to suddenly turn the pain level to 11.
It was just that the effects of medication were wearing off in the morning and his body was then able to react properly.

Post Acute Withdrawal Syndrome can last for up to a year, and sometimes longer. It definitely causes psychological issues like depression and anxiety, and can be a source of problems with cognition, or even physical symptoms like pain and balance issues. (Now, I only wait to see if he blames his current problems on PAWS.)
