We have no indication that Atlas and Oaken have ever been to a proper doctor, and the only reason they bothered to take Luna is because they could no longer deny that something was seriously wrong with her. Therefore they wouldn't need Medicaid for the boys and they probably get enough other money and benefits from just Luna that they don't need to add the boys.

It's only the raising of the eyebrows in such an extreme way that gets me. Maybe it's the boomerang that makes it look so strange like there's no inbetween where he's raising them: it's just down and to the sky, over and over. Not smiling to avoid wrinkles is insane, yes. (Just realized why: after he raises them here, it snaps back the beginning of the video where they're immediately down again. Ok, my mistake: it was the boomerang making him look cartoonish.)
