Their obligation is to keep their art hobby as a legitimate income/job that they can use to pay their bills. Most of them struggle to make rent every month. If they don't do a certain amount of tattoos each month, they're screwed and they'll have to stop tattooing or if they're independent they'll have to join a shop and have less control over what they do, as well as less money overall.

So that's the backdrop of what's going on in a tattooist's life.

No, they have no ethical obligations.

They don't want the client to be drunk, because that makes them bleed more and it's gross, so they'll turn someone away for that. They don't want the client to be on meth because they'll be annoying (but they'll complain about that later, they wouldn't turn someone away or accuse them of drug use). Most of them won't tattoo someone's face if it's their first tattoo or one of their first tattoos, but that's because they don't want to get a bad reputation in town when some mentally ill woman post-episode tells people that you ruined her life.

Everything they frame as an ethical obligation is for one of two purposes:

1) Trying to make sure they don't have a shitty few hours at work (since they never have a full day of work)
2) Trying to not eliminate opportunities to earn more money in the future

If their own underage child saves up enough money to buy a tattoo, the tattooist parent will gladly tattoo them. It's actually remarkable how little ethical concerns they have. It's really misplaced sociopathy; these people could be running chemical factories in India.
