I think these are two different things
Do we know where the package was sent from? Did they order it online and enter Ethan's address as the shipping address? Or did they order it themselves, and mail it to Ethan personally? In either case, I don't think UPS or Fedex's policy is a silver bullet here. I doubt they would've labelled it "actual real human skull (this package is a death threat)". It would surprise nobody here to learn that Ethan is exaggerating or bullshitting, but I don't see any reason to doubt that Bad Empanada and company would do this sort of thing.
